ABOUT: The “Connecting Communities – Social Prescription” Program aims to support members of the Fraser Coast community who may be socially isolated, lonely or who could benefit from social interaction as part of their care plan. Anyone referred to this program will be assigned a Link Worker or Volunteer Community Connector who will help them identify their interests and what they may like to do socially. The participant will then work closely with their connector to find local groups, events and activities that might be suitable. The program uses a warm referral approach to connect community members with suitable activities and groups that will in turn improve their quality of life and is not a case management service.
