Community transport for seniors and people with disabilities

Our TransComCare program provides subsidised community transport to older adults and people with disabilities and their carers to run important errands.

This may include taking clients to doctors, dentists, hospital appointments, shopping and social activities around the local district.

Eligible clients, carers or other agencies can register for assessment with our service by expressing their interest here.

Welcome letter

TransComCare is funded by the Department of Health and Aged Care and Department of Communities, Disability Services and Seniors.

The HBNC TransComCare service is delivered by volunteer drivers for low-care clients.

CHSP Manual 2023-2024 - 1.2.2 Entry-level support

The CHSP provides a small amount of services to help frail older people maintain their independence and continue living safely at home and in their communities.

The CHSP is not designed for older people with more intensive or complex care needs.  Clients who need ongoing high intensity care are outside the scope of this program.  People with higher needs can receive appropriate support through other aged care programs, such as the Home Care Package (HCP) program or residential aged care.

CHSP services delivered to a client should be lower than the subsidised cost of a Level 1 HCP (less than $9,000 per annum).
